Satisfaction survey

Surrey and Sussex Healthcare NHS Trust

This survey has been produced in order to measure our compliance with the Freedom of Information Act and will allow us to measure how well we respond to requests for information. The results of this survey will assist the Trust in reviewing and improving our own procedures.

The survey is anonymous and all completed responses will be directed to our Freedom of Information e-mail account. Response e-mails sent out to applicants will include a statement asking applicants to complete the survey on line? For those responses sent out by post, a hard copy of the survey will be attached to the response letter and will include a freepost envelope.

Quality of response
Purpose of this section is to review your recent request. Responses will be scrutinised and suggested improvements/areas of dissatisfaction will be investigated.

The survey will run from 1 April to 31 March each year. The results of the survey will be evaluated and the Freedom of Information Lead will present a report to the Information Governance Steering Group. The Freedom of Information policy and procedures will be reviewed in light of the results and any relevant amendments made. All the information you give will be treated in the strictest confidence and will only be used to monitor the Trusts Freedom of Information procedures. The results are held on a secure database according to the 1998 Data Protection Act and no individuals will be identifiable in any report.
